Health Department* *Position Title:* Community Health Nurse *Department:* Health & Family Services *Reports To:* Director of Health *Employment Status:* Full-Time, Permanent *Hours:* 35 hours per week (Monday?Friday; some evenings/weekends may be required) *Wage:* $47-54 per hour (to be determined based on experience and qualifications) *About the Role* Gwa?sala-?Nakwaxda?xw Nations is seeking a dedicated and compassionate Community Health Nurse to join our Health and Family Services team. The Community Health Nurse plays a key role in supporting community wellness through the delivery of culturally safe, holistic, and client-centered health services. Working under the direction of the Health Director, this position is responsible for delivering and supporting programs aligned with the Community Health Plan and ensuring high-quality care for individuals, families, and the broader community. This role includes both clinical and community-based nursing services, with a strong focus on prevention, health promotion, education, and chronic disease management. *Key Responsibilities* *Community Health & Clinical Care* * Provide holistic nursing care using assessment, planning, implementation, documentation, and evaluation of client care * Deliver community-based health clinics and home visits * Support prenatal, postnatal, newborn, and early childhood care * Provide chronic disease follow-up in collaboration with Home and Community Care staff * Support communicable disease control activities, including tuberculosis, STI prevention, and contact tracing * Manage immunization programs and seasonal vaccination clinics (e.g., influenza) * Provide contraceptive counselling and sexual health support *Health Promotion & Prevention* * Deliver health education and wellness programming for individuals, families, and groups * Support child growth and development monitoring and parenting education * Promote healthy living and injury/disease prevention throughout the community * Develop and update health education resources and materials *Community Programs & Coordination* * Organize and support community health events (e.g., Health Fairs, youth and family wellness initiatives, NNADAP-related programming) * Evaluate community health programs and recommend improvements * Act as a clinical resource for Community Health Representatives (CHR) and Home Support Workers * Collaborate with internal departments and external health partners *Documentation & Professional Practice* * Maintain accurate and timely clinical and administrative records using approved systems (e.g., Electronic Medical Records, Panorama) * Ensure confidentiality, privacy, and professional standards are maintained at all times * Follow all GNN Health policies, procedures, and code of ethics *Panorama System ? Responsibilities* * Manage and oversee access to personal health information within Panorama * Approve and review system access based on ?need-to-know? and least privilege principles * Conduct access audits and ensure compliance with privacy and security requirements * Support privacy and data accuracy processes and reporting * Ensure appropriate onboarding/offboarding of system users *Supervision* This position may provide guidance and oversight to: * Community Health Representatives (CHR) * Nurse Assistant(s) *Qualifications* *Education & Certification* * Current registration in good standing with the BC College of Nurses and Midwives (BCCNM) * Graduate of a recognized School of Nursing (RN or equivalent) * Current CPR-C certification * Current immunization certification *Skills & Abilities* * Strong clinical judgment and critical thinking skills * Excellent interpersonal, communication, and teamwork skills * Ability to work independently and within a multidisciplinary health team * Strong organizational and problem-solving skills *Knowledge & Experience* * Understanding of community health n- Location Port Hardy, BC
